Making an Appointment

To schedule an appointment, call our office at 419-586-5760 and set up your initial consultation or subsequent follow-up appointment. Press '1' for the front desk scheduling personnel. For your convenience, follow-up appointments can also be scheduled from the office at the end of your visit.

All appointments will be scheduled on a prioritized basis. Emergency and urgent issues are given same-day appointments when appropriate. In all cases, we will do our best to keep to your scheduled time. The surgeon will at times be obliged to attend to emergencies, either in person or via phone. We apologize when this may affect the accuracy of your visit time.

If you are unable to keep your scheduled appointment, please notify us at least 24 hours prior to your scheduled appointment so that we may accommodate other patients.

When You Arrive

To help ensure a smooth and comfortable visit, please read and follow these tips:

  • Please arrive approximately 15 minutes before your scheduled appointment time, so we can make sure your paperwork is complete and make copies of your insurance cards.
  • Please print, complete, and bring the below forms with you to your appointment. This way, you can complete the forms in a more comfortable environment and it will save you time on appointment day.

  • Insurance Card
  • Any relevant / required referrals
  • Co-payment/Deductible required by your insurance plan
Fees and Payment

The physician fee for a new patient is usually between $150 to $300. With the exceptions of Medicare and Public Aid patients, we require payment at the time of service for the office visit. Other services, such as x-rays, braces, casts, and other miscellaneous items may charged in addition to the professional fee. We also accept cash, Visa, and MasterCard.


To make it easier for our patients to get the care they need and deserve, we participate in as many insurance plans as possible.

For your convenience, your insurance claims will be filed by our insurance department. In some cases, your insurance reimbursement may not cover the full amount of services provided. Regardless of insurance payment, the balance of the charges remains your personal responsibility. If you do not have insurance, your account will be considered "self-pay," and payment will be due at the time of service.

Workers Compensation

We are Bureau of Workers’ Compensation (BWC) providers. Please bring all workers' compensation paperwork, including your claim number, card, and any other pertinent information, including primary insurance information, with you to your visit.

If you are experiencing a medical emergency, including active bleeding, a deformed or broken limb, etc., please call 911 or visit the closest emergency room immediately.

If you need our doctor after hours, please call your preferred hospital and have the doctor paged.
